Sports Physicals & Healthy Roster

 
A sports physical is only valid for one year from the initial examination. An up to date physical is REQUIRED for all athletes to be eligible to participate in any Mater Dei athletic activities. If the physical is expired, then the athlete will remain out of participation until a valid physical is provided. Below is the Physical Examination form and the Medical Eligibility form that must be completed by an MD, DO, PA, or NP. After both forms are completed, please upload it to your Healthy Roster account for our staff to review and clear your student athlete for athletic participation. 
Healthy Roster is an electronic medical record (EMR) documentation system that allows Mater Dei High School healthcare professionals to effectively document, communicate, and track any student injuries & illnesses. Healthy Roster is a web based and app based system that will allow parents to upload necessary documents, communicate through a messaging feature, and will keep them up to date with their students injury/illness status. This system encompasses the schools healthcare professionals, administrators, coaches, and parents into an all in one individualized student care circle. This system requires a one time registration for parents/guardians and will stay as an active account until your student graduates from Mater Dei High School in which the account will automatically become inactive.

  • All parents/guardians of returning & incoming students should have already received an email from [email protected] with instructions on how to create an account with Healthy Roster.
REGISTRATION INFORMATION
There are two parts to the registration for Healthy Roster which include 1) creating an account and 2) downloading the app
All of your students information should already be pre populated into the account, however there are a few extra steps that we are asking you do to successfully complete your students account. Please see the extra steps below:
  1. Make sure that all information (demographics) is up to date.
  2. Add an additional emergency contact if applicable so that we can have two on file.
  3. Add medical information including medical history and insurance information.
